The attached video is the program I use to make my questionnaire forms. I like how simple it is to use and build out. For those who want to link their form to their ad or calendar, Wufoo allows you to copy the embed code making this a seamless professional experience for the client.

*Note: When conducting your calls/interviews, before moving on to the next step, you must get a green light from the interviewee. In other words, if you are not satisfied that the client has given you a complete answer to your question,  you do not have a green light and should not move to the next stage of the close. 

One more thing, never forget that you are INTERVIEWING them, not the other way around. This is why I have provided these tools to help you remain in control of the call and how it proceeds.
