In order to partake in our Sales Team, we will need a few things from you first!

1. Please download Form W-9 and either email your completed Form W-9 back to us at support@foundationsforsuccess.com.

2. Along with sending your Form W-9, please also let us know which PayPal account email you would like your payouts sent to. (Payouts are only possible through PayPal.)

3. Once we have received your Form W-9 and PayPal account email, we will complete your partnership account and then notify you.

4. Please then confirm that your PayPal email is correct by visiting the My Profile tab. You will see your updated PayPal email towards the bottom of the form under the Additional Info section.

5. In the beginning of each year, we will send you a 1099-MISC for tax purposes.

IMPORTANT: Please take the time to confirm that your PayPal email address is correct so we may process your payouts without issue.
